Community Public Health Team

Community Public Health Team | 2024

The CORE Community Public Health Team (CPHT) is working with Herald Christian Health Center (HCHC) and the Los Angeles County Department of Public Health to address health concerns at the household level in South El Monte, El Monte, and Rosemead. 

We invite the community to participate in Household Health Assessments to understand health needs and get connected to care. We welcome members to join us creating community action plans for better health.

What is CPHT doing to support community public health in California?

We are working alongside HCHC to provide inclusive, contextually appropriate public health outreach to communities within the San Gabriel Valley.

 

Our goal is to focus on preventative measures that holistically address the causes of ailment which are specific to members of a community. This includes administering vaccinations, health screenings, lifestyle assessments, mental health, etc.

What should I expect if there is a Community Public Health Team in my community?

If you live in one of the service areas of El Monte, South El Monte, or Rosemead, you are eligible to receive services. We invite you to participate in the in-person household health assessment with a Community Health Worker (CHW). 

After the assessment, the CHW will offer to connect you to services based on your responses. Anyone living in or near the service area will also be invited to join community meetings to learn about the project and how to get involved.

Join our team as a community ambassador! Advocate for the concerns and needs of your community and local residents. Your involvement is crucial, and your community is counting on your participation.

What is a community ambassador?

An ambassador is a volunteer that supports their community by voicing health concerns and sharing health information to families and friends.
